1. I throw an incompletion.
2. There's a celebration in the endzone.
3. I apparently lose the game in OT on a safety off a play that never lines up.

That's an end of game bug. :( Game winning kicks have something similar. If the players are lined up in a screwed up manner you know the kick is going through and the game is over.
